Resolved [Macbook Pro]: Not booting properly, Crashes, Freezes, CPU problem?

first time posting on macrumors.
have technical issue i'd with:
tl;dr: computer freezes when more few basic tasks, , won't boot before several reboots. panic report seems indicate might cpu problem.
system info:
macbook pro, 15" ultimo 2011 (late 2011)
software: mac os x lion 10.7.5
--
processor: 2.2ghz core i7 (2675qm)
memory: 8 gb 1333 mhz ddr3 (upgraded 4 gb)
video cards:
-radeon hd 6750m - 512 mb gddr5 sdram
-intel hd graphics 3000 - 384 mb ddr3 sdram
hard drive: 500 gb (5400 rpm), serial ata (6 gb/s)

machine bought new in 2011.
-this makes me quite warranty has expired now.
problem:
problem started approximately 1 week ago , has been there consistently ever since:
1: not boot properly: when start machine, hear startup chime, see apple logo, , machine loads does. however, when apple logo disappears, screen becomes white , stays way until manually turn off holding down power button. turns off after 5 minutes.
-sometimes system boots properly, , i'm able use computer (which how i'm writing post in first place).
2: after time, machine spontaneously freezes, , have turn off manually, because i'm unable interact it. often, shows either row of blue , black vertical lines across screen, or black , white ones. screen freezes without optical glitches.
-i've noticed tendency of freezing highest when opening , using decently heavy programs or using computer more few basic tasks. (examples: browsing google chrome, playing music spotify, etc.)
3 of many times these problems have occurred during last week, machine asked me manually restart computer. afterwards opened panic/problem report, of 1 of them can found here via pastebin link:

-the report seems indicate might cpu problem, far can tell.
what i've tried:
followed of support articles on support.apple.com , tried these solutions:
booting in safe mode. (shift key @ start-up).
-this did not solve issue.
running aht @ start.up (apple hardware test) (d key @ start-up)
-i ran basic test, stated no trouble found
booting in single-user mode , verbose mode (cmd + s , cmd + v respectively)
-none of these solved issue
-i did not change files in single-user mode.
booted in recovery mode (cmd + r) , reinstalled lion. did not either.
potential causes:
-during summer had computer lying on bed. sled off onto floor, suppose might have caused internal damage. however, happened quite few weeks before issue began, far can recall.
-i read online fans might have collected dust on time, can make cpu shut down prevent damage overheating.
-i recall reading gel inside computer there wrong with.

you might have problem radeon hd 6750m video card, known problem. see macbook pro repair extension program video issues.
may need make appointment have tested @ apple store.
