Dead hdd (probably) upgrading to ssd?


hi there
while i've thought upgrading hard drive came along macbook pro 13" (2011 late) ssd, never came outside thoughts.. yesterday when playing around on (running yosemite), froze, , after waiting 5 minutes non responding mbp, decided shut down holding down power button - resulted in mbp unable boot (it let me go through login scene loading bar stuck somewhere halfway , automatically turn off after 5-10 minutes. ran through steps concerning problem, unfortunately nothing seemed work out me... stupidly freaked out in disk utility while booted in recovery mode, , hdd won't mount, repair, format , can't chosen boot (mbp shows blinking folder question mark inside when trying boot). seems totally dead me, though ain't no expert on subject.
i'm confused , have bunch of questions needed answered , appreciate help, because i'm kinda helpless...

1. how sure can replacing hdd new ssd solve problem?

2 . saying replacement solve it, enough format ssd mac os extended (journaled) , make bootable usb yosemite installation install from? or there else need do?

3. if had choose between samsung 850 evo or crucial bx100 (both 500gb editions) go mbp mine?

put in buying tips , advice, guess makes more sense here.. dunno how delete first thread through :(

what did do? possible drive not dead did longer mounts, don't know.

either way, ssd should you, , if doesn't rebox , return it.
